Key Responsibilities

• Provide occupational therapy services to patients within a home health setting across a designated service area
• Maintain a minimum productivity expectation of 30 points per week
• Complete all clinical documentation by 10:00 AM the following day
• Utilize HCHB (Homecare Homebase) for documentation and care coordination
• Travel to patient homes within the assigned coverage radius

Required Qualifications

• Active Occupational Therapist (OT) license in the state of Washington
• Minimum of one (1) year of home health experience required
• Proficiency with HCHB (Homecare Homebase) EMR system
• Must be available to cover a 50+ mile radius from the office location

Preferred Experience / Skills

• Experience managing a high-volume caseload in a home health environment
• Strong time management and documentation skills to meet next-day charting requirements
