divh2Healthcare Facility Nurse/h2pThis role is based at a healthcare facility in the Bronx, supporting admissions, care planning, and overall nursing department operations while ensuring high-quality resident care./ppAdmissions experience is a must!/ppDuties and Responsibilities:/pulliConduct admission assessments for new residents and hospital returns; initiate care plans./liliReview PRI documentation and coordinate all aspects of the admission process with the interdisciplinary team./liliAssist in the development, implementation, and oversight of nursing policies, procedures, and department operations in collaboration with the Director of Nursing (DON)./liliSchedule and assign nursing staff to effectively meet resident care needs./liliFoster a compassionate environment by supporting residents and their families./liliProvide information and respond to questions from residents, families, and staff./liliReview and revise resident care plans based on changes in condition./liliParticipate in discharge planning as needed./liliComplete MDS, charting, and all required documentation in accordance with policies and procedures./liliAudit resident and departmental records to ensure compliance with facility standards and regulatory requirements./liliMaintain a safe, clean, and compliant working environment by adhering to established guidelines./liliEnsure confidentiality and proper handling of resident information in accordance with HIPAA and corporate policies./liliMaintain nursing supply inventory and related operational needs./liliSupervise or direct resident care as assigned./lipRequirements/Qualifications:/pulliActive, unrestricted New York State Registered Nurse (RN) license./liliLong-term care (LTC) experience required; admissions experience needed./liliStrong clinical assessment and critical thinking skills./liliAbility to make sound, independent decisions./liliDemonstrated leadership and supervisory abilities./liliBasic computer proficiency; familiarity with Microsoft Office (Excel, Word, Outlook) preferred./liliExcellent communication and interpersonal skills./liliStrong team player with the ability to collaborate across departments./li/ul/ul/div
