Key Highlights Weekly pay: $1,910 – $1,965 per week Guaranteed hours: 36 per week Contract duration: 8 weeks Shift: Full-Time with weekend coverage Start date: 08/31/2026 Travel assignment in an inpatient, multidisciplinary care environment Position urgency: Quick Start Employer: Healthforce Healthcare LLC Responsibilities Develop, implement, and evaluate case management plans for patient populations, coordinating care across disciplines Advocate for patients and families; communicate and collaborate with physicians, nurses, social workers, and payers to optimize care Document Case Management plans clearly, including all patient contacts and care decisions Facilitate appropriate utilization and discharge planning; participate in interdepartmental meetings related to Case Management/Social Services Collaborate with payers and review organizations to ensure proper resource use and adherence to regulatory requirements Maintain current knowledge of discharge planning, utilization processes, and case management best practices through ongoing education Requirements Current, unrestricted Registered Nurse license in Colorado Bachelor’s Degree in Nursing; Master’s degree preferred At least three (3) years of clinical experience in a hospital setting and at least five (5) years of case management experience in a hospital setting Case management certification preferred or willingness to obtain within two (2) years of employment Ability to document and communicate clearly; strong collaboration with physicians, patients, families, and staff Willingness to fulfill weekend coverage and participate in the described weekend rotation Benefits and Support Guaranteed hours: 36 per week 8-week contract duration with a travel assignment structure Direct support from a dedicated recruiter for the assignment Access to assignment-related resources and coordination throughout the engagement #J-18808-Ljbffr HealthForce Healthcare
