Home Health Physical Therapist

Seva Home Rehabilitation is seeking a compassionate, reliable Physical Therapist to provide skilled in-home therapy services to patients who are homebound, recovering from illness or injury, managing chronic conditions, or recently discharged from a hospital, rehabilitation center, skilled nursing facility, or other medical facility in Orange County, FL. This is a 1099 independent contractor position, not a direct hire/employee role. You manage your own schedule and caseload. Compensation is discussed directly during your phone interview, as rates vary by clinician, experience, and assignment. Evaluate patients' mobility, strength, balance, functional ability, pain, fall risk, and rehabilitation needs in the home setting. Develop, implement, and update individualized physical therapy plans of care consistent with physician orders. Provide skilled interventions including therapeutic exercise, gait training, transfer training, balance training, fall-prevention education, and patient/caregiver instruction. Establish measurable rehabilitation goals and assess patient progress. Coordinate care with physicians, nursing staff, occupational therapists, speech-language pathologists, and therapy assistants. Complete evaluations, reassessments, visit notes, progress reports, and discharge summaries accurately and within required timeframes. Maintain patient confidentiality and comply with HIPAA and applicable Florida regulations. Required Qualifications Active, unrestricted Florida Physical Therapist license. Graduation from a CAPTE-accredited physical therapy program or equivalent meeting Florida licensure standards. Current CPR certification or ability to obtain before providing patient care. Valid driver's license, reliable transportation, and automobile insurance. Ability to pass Level 2 background screening. Strong clinical judgment, communication skills, and a patient-centered approach. Preferred Qualifications One or more years of home health, outpatient, acute-care, rehabilitation, or geriatric therapy experience. Experience with OASIS-related workflows and Medicare home-health documentation. Bilingual ability is a plus. #J-18808-Ljbffr
