• Shift: Day shift, 5x8-hour shifts (08:00-16:30), Monday-Friday
• Hours/Week: 40 hours
• Employment Type: Per Diem / Contract
• Licensing/Certifications: Indiana License or Pending, BLS required
• Experience: 1+ year clinical experience preferred, new grads accepted

Pay & Benefits:

• Hourly Pay Rate: $54.12/hour

What You'll Do:

• Provide outpatient physical therapy to diverse patient populations
• Develop and implement individualized treatment plans to improve patient outcomes
• Collaborate with healthcare team to ensure continuity of care
• Educate patients and families on exercises and recovery strategies
• Document patient progress accurately using Cerner charting system
• Participate in occasional on-call duties, including minor Saturday hours
• Float within scope to meet facility needs as required

What You Need:

• Valid Indiana state license or pending license with eligibility to practice
• BLS certification (required)
• Minimum 1 year of outpatient physical therapy experience preferred
• Strong communication and patient management skills
• Ability to work independently and as part of a team
