Nurse Practitioner - Per DiemAdira at Riverside, a luxury skilled nursing facility in Yonkers, New York, is seeking a Per Diem Nurse Practitioner to join our team. As a Nurse Practitioner, you will work collaboratively with our medical director and attending physicians to provide high-quality medical care to our residents. If you are a compassionate and skilled healthcare professional with a passion for delivering exceptional patient care, we encourage you to apply!Responsibilities:Assess, diagnose, and treat residents according to your scope of practice and NYS LawTake and review patients' medical histories, conduct physical examinations, and write admission and follow-up notes in the EMROrder and interpret labs and diagnostic studies, diagnose and treat medical conditions, and make referrals when neededPrescribe medications, including controlled medicationsParticipate in the development and implementation of individualized care plansCollaborate with the interdisciplinary team to provide comprehensive care to residentsRequirements:Current nursing and advanced practice license in compliance with State lawCurrent DEA licensureAbility to develop and maintain collaborative relationshipsExperience in working with older adults with complex medical conditionsAbility to assess subtle changes in health conditions of the older adultWorking knowledge of State and Federal Long Term Care regulationsExcellent administrative, organizational, and verbal skillsEffective communication skills with the geriatric populationAbility to work independentlyDetail orientedDependable and reliableProficiency in EMR systems (Sigma/MatrixCare a Plus)Preferred Qualifications:Clinical background in adult, family, or geriatric practicePast experience working in a nursing home or with older adults in other settingsExperience in working in a post-acute settingWork Schedule:Every WednesdaySalary:$50 - $80 HOURLYJoin our team and experience the luxury and style of Adira at Riverside!
