This person will strengthen the relationship between the Home Care Department and the Interdisciplinary Team (IDT) and will provide hands‑on leadership in all aspects of home care. The HCS will develop partnerships with families and caregivers, and may be required to make in‑home visits for assessment and ongoing case management. The position includes a weekend/on‑call rotation. Location and Schedule Homestead, PA Schedule: 7:30 a.m. – 4 p.m., Monday‑Friday with on‑call rotation plus as needed based on participant care. Program approved by the National Board for Certification in Occupational Therapy. Physical Therapy Assistant program approved by the American Physical Therapy Association. Required Experience 5 years of direct care experience with at least 1 year working with the frail and elderly population; 2 years of supervisory experience; experience teaching patients and other health care workers preferred. Required Certifications / Licensure Valid Pennsylvania COTA/L, PTA, CNA or LPN license. Valid PA Driver’s License. Required Skills & Abilities Knowledge of the medical, social, and emotional needs of a frail, elderly population. Strong interpersonal relationship skills. Demonstrated ability to effectively collaborate with others. Proactive approach. Working knowledge of durable medical equipment (DME). Crisis intervention skill. Effective written and oral communication skills. Strong organizational and planning skills; ability to manage multiple priorities. Ability to work independently and to utilize critical decision‑making skills. Working knowledge of utilization review, quality assurance and managed health care concepts. Ability to work with the interdisciplinary team approach to care for the elderly. Working knowledge of local health care and geriatric service networks. Basic working knowledge of Windows operating systems, e‑mail, word processing. Ability to deliver services in a compassionate, responsive, and courteous manner. Ability to work effectively with staff, participants, providers, and referral sources. Interest in geriatrics and community‑based programming.
