PRN Physical Therapist (PT) Location: Maple Heights, Ohio Facility: Phoenix of Maple Heights Position Summary Phoenix of Maple Heights is seeking a motivated and compassionate PRN Physical Therapist (PT) to join our rehabilitation team. This role provides flexible, as‑needed coverage to support patient care and therapy services. The ideal candidate is committed to delivering high‑quality, patient‑centered care and working collaboratively within an interdisciplinary team. Key Responsibilities Evaluate residents/patients and develop individualized treatment plans based on clinical findings Provide skilled physical therapy services in accordance with physician orders and established care plans Document evaluations, progress notes, and discharge summaries using Net Health and PCC systems Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes Collaborate with nursing staff, physicians, and other therapy team members to ensure coordinated care Educate patients and families on therapy goals, home exercise programs, and safety measures Maintain compliance with all federal, state, and facility regulations and standards Qualifications Current license as a Physical Therapist in the state of Ohio Graduate of an accredited Physical Therapy program Strong clinical assessment and treatment skills Experience with Net Health and PCC preferred Excellent communication and interpersonal skills Ability to work independently and adapt to varying patient needs Previous experience in a skilled nursing or rehabilitation setting preferred Schedule PRN / As‑needed basis Flexible scheduling based on facility needs Why Join Phoenix of Maple Heights Supportive and collaborative work environment Opportunity to make a meaningful impact on patient recovery and quality of life Flexible schedule to match your availability Phoenix of Maple Heights is an equal opportunity employer. #J-18808-Ljbffr Phxhealthohio
