PT - Physical Therapist

Home Health Physical Therapist / Occupational Therapist / Speech Language Pathologist Assignment Duration This is a 13-week contract position. Schedule Monday to Friday, 5 shifts per week, 8 hours per shift. Job Description We are seeking experienced Home Health Physical Therapists, Occupational Therapists, or Speech Language Pathologists to provide care in the home health setting. The provider will cover territories including Burnet, Bertram, and Lampasas, and may be required to assist in other areas within a 60-mile radius. Responsibilities Conduct an average of 4-6 visits per day based on the type of therapy provided. Manage productivity metrics achieving 28-32 points per week across various visit types. Document patient evaluations, treatment plans, and ongoing assessments via Homecare Homebase. Participate in weekly case conferences to discuss patient progress and care plans. Maintain compliance with all home health regulations and documentation standards. Required Experience / Certifications / Licensure Home Health and OASIS experience required. Homecare Homebase documentation experience preferred; candidates without this experience may be considered if willing to undergo training.
