PSR/Medical Assistant- Wellness

Position: Outreach Coordinator - Wellness Team

Overview:

The Outreach Coordinator supports the Wellness Team by engaging patients in preventive care initiatives, with a primary focus on senior populations. This role is responsible for coordinating patient outreach efforts, scheduling wellness visits, and promoting timely access to care. The ideal candidate is detail-oriented, patient-focused, and comfortable working in a fast-paced, outreach-driven environment.

Key Responsibilities

Patient Outreach & Engagement

• Conduct outbound calls to patients to promote wellness visits, annual physicals, and preventive care services

• Engage patients with professionalism and empathy to encourage participation in care programs

• Support organizational initiatives aimed at improving patient engagement and health outcomes

Scheduling & Care Coordination

• Schedule patient appointments for wellness checks, physicals, and specialist visits as needed

• Coordinate follow-up appointments and ensure continuity of care

• Collaborate with internal teams to align scheduling and outreach efforts

Data Entry & Documentation

• Accurately document patient interactions and outreach activities within electronic medical record (EMR) systems

• Maintain up-to-date patient records and ensure data integrity across systems

• Track outreach outcomes and support reporting efforts as required

Required Skills

• Strong patient service and customer service skills

• Effective communication and interpersonal abilities

• Organizational and project coordination skills with the ability to manage multiple tasks simultaneously

• Proficiency in Microsoft Outlook and EMR systems

Preferred Qualifications

• Experience with outbound patient outreach in a healthcare setting

• Familiarity with patient scheduling and appointment coordination

• Prior experience working with electronic medical records (EMR)

Pay and Benefits

The pay range for this position is $21.00 - $21.00/hr.
