Schedule
- Shift: 8-hour days
- Weekends: Not required

Assignment Details
- Patient Ratio: 1:1
- Typical Caseload: Up to 6 patient visits per day
- Guaranteed Hours: Facility may cancel up to one shift per pay period

Requirements
- Minimum 1 year of skilled home health physical therapy experience
- BCLS (Basic Cardiac Life Support) certification required
- Oasis experience required
- Pending license accepted

Roles & Responsibilities
- Provide physical therapy services to patients in a home health setting.
- Develop and implement personalized treatment plans based on individual patient needs.
- Educate patients and their families on home exercise programs and rehabilitation techniques.
- Monitor patient progress and adjust treatment plans as necessary.
- Collaborate with other healthcare professionals to coordinate patient care.
