Title: Licensed Practical Nurse

Employment Type: Contract (3+ Months)

Compensation Range: $36.00–$40.00 per hour

Location: Liberty, New York

Work Schedule: Onsite; 7:00 AM–3:00 PM or 3:00 PM–11:00 PM, 5 days per week, every other weekend; overnight availability preferred

Position Summary

The Licensed Practical Nurse provides professional nursing care to residents in accordance with individualized care plans under the direction of a Registered Nurse. The position requires sound clinical judgment, effective communication, accurate documentation, and supervision of Nursing Assistants while ensuring quality resident care.

Key Responsibilities

• Provide professional nursing care according to individualized care plans.
• Administer prescribed medications and therapeutic treatments in accordance with physician orders.
• Observe, assess, and document resident conditions and report changes appropriately.
• Measure and record vital signs.
• Collect laboratory specimens as required.
• Maintain accurate nursing documentation and medical records.
• Communicate effectively with physicians, Registered Nurses, residents, and families.
• Assist with resident admissions, transfers, and discharges.
• Supervise Nursing Assistants and assigned clerical personnel.
• Ensure compliance with facility policies and applicable nursing standards.

Required Qualifications

• Active Licensed Practical Nurse (LPN) license in New York State.
• Ability to provide direct resident care in a long-term care setting.
• Knowledge of medication administration and nursing procedures.
• Strong communication and documentation skills.
• Ability to supervise support staff.
• Ability to work every other weekend.

Preferred Qualifications

• Previous skilled nursing or long-term care experience.
• Experience with electronic medical records.
• Strong assessment and clinical documentation skills.
• Flexible availability, including overnight shifts.
