RN Case Manager - Travel Position

Schedule

8:00 AM - 4:30 PM; Every other weekend required

Job Description

We are seeking an experienced RN Case Manager for a travel position. The ideal candidate will possess acute (hospital) case management experience and have working knowledge of Meditech, Midas, and Interqual. This role is critical in providing exceptional patient care and coordinating care plans for individuals with complex health needs.

Responsibilities

• Conduct comprehensive assessments of patients' needs and develop individualized care plans.
• Coordinate with healthcare professionals to ensure effective care delivery.
• Monitor patient progress and update care plans as needed.
• Advocate for patients and support them through the healthcare system.
• Ensure compliance with hospital policies and regulations.

Required Experience / Certifications / Licensure

• Valid Registered Nurse (RN) license.
• Experience in acute hospital case management.
• Proficient in Meditech, Midas, and Interqual.

Assignment Duration

This is a 13-week contract position.
