Behavioral Health Care Coordinator

Health Connect America, Inc. seeks an AL professional to support delivery of behavioral health, counseling, and therapeutic services to children, adolescents, adults, and families. In this role, you’ll help coordinate in‑home, in‑office, and community‑based mental and behavioral healthcare, ensuring services are timely, well‑documented, and client‑centered. You will collaborate with multidisciplinary clinicians, community partners, and agencies, assist with referrals and scheduling, maintain accurate records, and uphold compliance and confidentiality standards while supporting our mission of accessible, compassionate care across local communities.
