Guaranteed Hours: 40 Contract Weeks: 14

Requirements

• Current State License as a Physical Therapist - Required
• Proof of current CPR certification - Required
• Current driver's license with reliable transportation and insurance - Required
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an accredited program - Required

Responsibilities

• Perform clinical assessments to identify patient needs and appropriateness.
• Document and complete Plans of Care, progress notes, and other clinical documentation.
• Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders.
• Assist and instruct patients and families on transfer techniques and equipment use.
• Provide education regarding therapeutic procedures based on patient demographics and readiness.
• Adhere to infection control and universal precautions in patient care activities.
