Youth Services Case Manager — Field-Based LifeSet

CarePlus New Jersey seeks a Lifeset Specialist to support 8–10 young adults, ages 17–22, as they transition to adulthood in a community-based program across Bergen, Passaic, Hudson, Morris, and Essex counties. You will provide in-home and community services, meet weekly with each youth, document all contacts, help set goals, and collaborate with supervisors to develop and adjust plans. #J-18808-Ljbffr Care Plus NJ, Inc.
