MINIMUM QUALIFICATIONS

Must be able to drive for long periods of time and board flights with minors to other states. Maintain a satisfactory driving record which insures maintenance of liability insurance coverage. Possess a valid California Driver's License, motor vehicle and auto insurance. Education: Bachelor’s degree in psychology, sociology or related field. Other: Attainment of at least 21 years of age.

JOB DESCRIPTION

The Intake Coordinator is responsible for managing and implementing the referral and intake process for the program, coordinating the marketing of FFA open beds in the local and surrounding communities, and work with the foster care team on recruitment, support, and retention of foster families. Transport UC to fly out location and chaperone UC via airfare to destination.

ESSENTIAL FUNCTIONS

Drive vehicle to designated route, picking up and discharging UC in accordance with established time schedules; observe legal and defensive driving practices; assure compliance with applicable traffic and transportation laws, codes, and regulations. Maintain order and discipline among children on the vehicle, airplane, and stops/waiting areas in accordance with the program policies related to disciplining UC. Maintain your vehicle up to standard for use. Be available to travel and complete intake 24/7 365 days a year as needed. Adhere to established procedures related to the intake and release of UC. Transport UC at scheduled departure and arrival times as assigned. Must be able to board/chaperone minor in flights as needed. Remain awake during work hours to ensure the safety of the minors. Determine appropriate action in potential or real medical emergency situations according to established guidelines; administer first aid as needed; conduct routine emergency evacuation drills as assigned. Prepare and maintain records and reports related to minors as needed. Communicate with foster parents and Supervisors to resolve issues or concerns as needed. Attend safety meetings and programs as assigned. Perform related duties as assigned. Complete an initial placement match assessment of the children to determine the appropriateness of the placement and the best fit with a foster home. This includes presenting behavioral issues and medical problems. Contact appropriate staff to identify if children are an appropriate match for a certain foster home. Then facilitate contacting the foster parent to see if they will accept the placement. Coordinate with the placing agency representative to set up the placement. Make sure placement paperwork is signed, completed and correct at the time of placement, including intake form, foster care agreements, authorization forms, medical forms, and children’s photograph. Coordinate where intake placement will take place and who is to be present. Provide transportation if needed. The Intake Coordinator will coordinate with the Office Supervisor for program’s staff to be present. In the latter case, the Intake Coordinator will monitor the completion of the intake paperwork. The coordinator is on-call via agency cell phone twenty-four hours a day, seven days a week for after hours, weekend, and emergency placements.
