30 70 cases between personal provision and medical direction
Medical direction of CRNAs in no more than 1:4 ratio
Ortho, neuro, healthy peds, non-OR anesthesia, L&D coverage
Hearts if fellowship-trained
All cases except transplants and complicated pediatrics
