Overnight Caregiver - High-Acuity Home Care

Job Summary:

We are seeking an experienced Overnight Caregiver - High-Acuity Home Care to join our team at SYNERGY HomeCare Of The West Valley in Goodyear, Arizona. As an Overnight Caregiver, you will provide exceptional care to our clients who require high-acuity care, ensuring their safety, comfort, and well-being during the overnight hours. Responsibilities:

• Provide one-on-one care to clients who require high-acuity care, including assistance with activities of daily living (ADLs), medication management, and medical conditions

• Monitor clients' conditions and report any changes to the healthcare team

• Collaborate with clients' families and healthcare providers to develop and implement personalized care plans

• Maintain accurate and detailed records of client care and services provided

• Ensure a safe and clean environment for clients, including administering medications and managing medical equipment

• Provide emotional support and companionship to clients, promoting their mental and emotional well-being

• Develop and maintain positive relationships with clients, families, and healthcare providers

Requirements:

• High school diploma or equivalent required; degree in healthcare-related field preferred

• Minimum of 1 year of experience in healthcare or caregiving; experience with high-acuity clients a plus

• Ability to work independently and make sound decisions in a fast-paced environment

• Excellent communication, organizational, and time management skills

• Compassionate and empathetic approach to care, with a strong commitment to client-centered care

• Ability to work flexible hours, including overnight shifts and weekends

• Reliable transportation and valid driver's license (if applicable)
