Registered Nurse (RN) – Part-TimeSequoia Transitional Care – Porterville, CA Pay Range: $40.00 – $45.00 per hour Status: Part-TimeGeneral PurposeThe primary purpose of the Registered Nurse (RN) position is to supervise the day-to-day nursing activities of the facility during the assigned shift. The RN ensures that nursing services are provided in accordance with federal, state, and local regulations and facility policies to maintain the highest level of quality care for residents. This role works closely with the Director of Nursing (DON) and Assistant Director of Nursing (ADON) to support safe, effective, and compassionate care.Administrative FunctionsAssist the Director of Nursing in directing the daily nursing operations of the facilityEnsure compliance with facility policies, procedures, and regulatory standardsParticipate in the development and implementation of nursing policies and proceduresComplete required reports, charting, documentation, and medical forms accuratelyAssist with admission, transfer, and discharge processes for residentsParticipate in facility surveys conducted by regulatory agenciesMaintain confidentiality of resident information and Protected Health Information (PHI)Nursing Care FunctionsProvide direct nursing care and clinical supervision to nursing staff during assigned shiftMonitor resident conditions and report significant changes to physicians and leadershipAdminister medications and treatments as ordered by physiciansMonitor medication passes and treatment schedules to ensure compliance with physician ordersReview nurses' notes and documentation for accuracy and completenessConduct rounds to observe resident condition and ensure quality of careAssist in managing seriously ill residents and coordinating care with physiciansNotify physicians and family members of changes in resident conditionEnsure residents receive appropriate care in accordance with their individualized care plansPersonnel & Leadership FunctionsSupervise nursing staff including LVNs and Certified Nursing Assistants during the shiftEnsure adequate staffing levels to meet resident care needsDelegate assignments appropriately and monitor staff performancePromote teamwork and maintain a positive work environmentAssist with staff coaching, education, and performance evaluationsEnsure staff follow facility policies including infection control and safety proceduresCare Planning & AssessmentParticipate in the development and review of resident care plansEnsure nursing staff follow care plans when providing resident careCollaborate with the interdisciplinary team to ensure coordinated resident careAssist with resident assessments and documentation as requiredSafety & Infection ControlEnsure compliance with infection prevention and control protocolsMonitor staff adherence to proper hand hygiene and PPE usageMaintain a clean, safe, and sanitary environment in nursing work areasParticipate in safety, infection control, and emergency preparedness programsQualificationsGraduate of an accredited nursing programMinimum two years of nursing experience preferred, ideally in a skilled nursing or long-term care settingExperience with rehabilitative and restorative nursing practices preferredCurrent, active Registered Nurse (RN) license in the State of CaliforniaCurrent CPR certificationExperience with PointClickCare (PCC) preferredMust maintain all required continuing education and licensing requirementsSkills & AbilitiesStrong leadership and supervisory abilitiesAbility to make independent clinical decisions when necessaryAbility to communicate effectively with residents, families, staff, and physiciansKnowledge of long-term care regulations and nursing standards of practiceAbility to maintain professionalism in high-stress situationsPhysical DemandsAbility to move throughout the facility during the workdayAbility to lift, push, or pull up to 25 poundsAbility to assist residents when necessaryAbility to stand, walk, bend, and reach throughout the shiftWork EnvironmentThe RN works throughout the facility including nursing stations, resident rooms, treatment areas, and clinical workspaces.
