Hospice RN Case Manager
Optimal Care
Optimal Care seeks a compassionate Hospice RN Case Manager to coordinate end-of-life care for patients at home and in facilities. You will manage a focused caseload, perform comprehensive assessments, create and update care plans, and deliver expert symptom and pain management. The RN collaborates closely with an interdisciplinary team, educates families, and provides emotional support. Responsibilities include accurate EMR documentation, regulatory compliance, participation in on-call rotation, and advocacy for patient comfort, dignity, and goals of care. $10,000 sign-on bonus offered.
