Social Worker - LCSW

Case Manager
Coordinates management of care and ensures optimum utilization of resources. Oversees service delivery and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment and care planning. Implements care plans and evaluates individual patient needs. Enhances the quality of patient management and satisfaction. Promotes continuity of care and cost effectiveness. Integrates functions of case management, utilization review, and discharge planning. Serves as a patient advocate through resource utilization and discharge planning. Addresses the holistic needs of the patient.

End Date: 12/19/2026
Duration: 12 Weeks
Shift: Monday - Friday, 8:00 AM - 4:30 PM
Experience: Minimum 1 year of Case Management and discharge planning experience in a hospital setting
License: California SW license required
Certifications: BLS certification required (must be through the American Heart Association)
Must-Have: Must attach California SW license and BLS certification to submission
