This role is ideal for a provider who thrives in a supportive, forward-thinking environment and is committed to whole-person care. You will provide comprehensive medical services, foster lasting relationships with your patients, and collaborate with a high-functioning care team to drive optimal health outcomes. Compensation : Guaranteed salary with earning potential of $300k+, relocation offered WHAT WE OFFER:

RESPONSIBILITIES AND DUTIES:

* Provide direct patient care through office visits, telehealth, and occasional same-day appointments * Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions * Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions * Educate and empower patients on preventive health and functional medicine principles * Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff * Document visits in the EHR in a timely and compliant manner * Participate in team meetings, quality improvement initiatives, and training sessions * Support clinic operations by contributing to a positive, team-oriented environment

REQUIRED QUALIFICATIONS

* Active Oregon license as a Certified Physician Assistant ( or eligibility to obtain ) * Board certification in Family Medicine or Internal Medicine * Strong interest in preventive, functional, and lifestyle medicine * Excellent communication and patient relationship skills * Comfortable working in a fast-paced, collaborative environment * Proficient in using electronic health records
