LPN Care Coordinator

Working remotely on a full-time basis, the LPN Inbound Care Coordinator will ensure high-quality care services by managing patient care plans, coordinating with clinicians and patients, and maintaining effective communication to enhance healthcare outcomes. Key responsibilities: Complete individualized patient care plans and perform care management using the electronic medical record system Maintain frequent contact with patients to provide support and manage compliance with care management programs Document all client communications accurately and keep the Team Supervisor informed of pertinent issues Required qualifications: Must be a Licensed Practical Nurse (LPN) Ability to work a weekend schedule and specific weekday hours Proficiency in using computers and Microsoft Office programs Compassionate and empathetic with exceptional customer service skills Ability to multitask and thrive in a fast-paced environment
