Full-Time Home Health Case Manager RN

Full-time Home Health Case Manager RN provides comprehensive nursing care and case management in patient homes. The role coordinates interdisciplinary teams, develops individualized care plans, and ensures compliance with state and federal regulations.

Key Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans
• Coordinate and manage patient care across the continuum, ensuring quality and efficiency
• Provide skilled nursing care in accordance with physician orders and established protocols
• Monitor patient progress and adjust care plans as necessary
• Educate patients and families on disease management, medications, and care techniques
• Collaborate with physicians, therapists, social workers, and other healthcare professionals
• Maintain accurate and timely documentation in compliance with regulatory standards
• Ensure compliance with all state, federal, and agency guidelines

Qualifications:

• Active Registered Nurse (RN) license in the state of Idaho (or compact license)
• Associate or Bachelor of Science in Nursing (ASN or BSN) required
• Minimum of 12 years of nursing experience; home health or case management experience preferred
• Current BLS (Basic Life Support) certification
• Valid driver's license and reliable transportation
