Registered Nurse Home Health

University Home Care Inc is seeking a compassionate and dedicated Registered Nurse Home Health to join our team in Livonia, MI. This role involves providing high-quality nursing care to patients in their homes, promoting health, managing chronic conditions, and supporting recovery. The ideal candidate will demonstrate excellent clinical skills, strong communication abilities, and a commitment to patient-centered care.

Responsibilities

Provide skilled nursing care to patients in their homes according to physician orders and care plans.
Assess patients' health status and needs, develop individualized care plans, and document progress.
Administer medications, treatments, and therapies as prescribed.
Educate patients and their families on disease management, prevention, and wellness.
Collaborate with interdisciplinary healthcare team members to coordinate care.
Maintain accurate and timely clinical documentation and comply with all regulatory requirements.
Monitor patient safety and report any changes or concerns promptly.

Requirements

Current valid Registered Nurse (RN) license in the state of Michigan.
Minimum of 1 year of nursing experience, preferably in home health or related field.
Knowledge of home health care regulations and nursing best practices.
Strong assessment, clinical, and organizational skills.
Excellent interpersonal and communication skills.
Reliable transportation to travel to patient homes in Livonia, MI area.
