Job Overview VIVA Pediatrics is a home health services company with a specialty in private duty nursing, therapy and skilled nursing visits for infants and children. Position PDN RN/LVN: 4 y/o (GB/Trach) Responsibilities Assess home health patients to identify the physical, psycho‑social, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations. Implement, develop, and document the plan of care to ensure quality and continuity of care. Provide care utilizing infection control measures to protect both staff and patients, ensuring compliance with OSHA and state, federal, and local regulations. Comply with all VIVA Pediatrics policies and procedures and promote the agency philosophy. Communicate effectively with patients/families, team members, and other health‑care professionals, as evidenced by clinical notes, case conferences, communication notes, and evaluations. Notify supervising nurse of changes in the client’s condition immediately. Provide health education to clients and/or caregivers. Document interim changes of the plan of care in a timely fashion. Location & Schedule Location: Weatherford, TX 76086 Job Type: Full‑Time / Part‑Time / PRN Schedule: Sun/Thurs/Sat (8:00 PM – 8:00 AM) Skills: GB / Trach Compensation LVN: $22–$28 per hour; RN: $30–$40 per hour. Requirements At least 1 year of work experience, or 6 months of pediatric home health experience as a licensed RN or LVN. CPR Certification. Valid driver’s license. Benefits Paid time off.
