Job Overview PDN RN/LVN: 6 y/o (GB/Trach/Vent) (7A-7P) We are seeking a Pediatric Home Health Nurse, RN or LVN, to provide one-on‑one skilled nursing care to medically fragile children in the home setting in City, ZIP. Full‑Time, Part‑Time, and PRN opportunities available. Case Information Location: Fort Worth, TX 76140 Job Type: Full‑Time / Part‑Time / PRN Skills: G‑Tube/Trach/Vent Schedule: Sat/Mon (7A-7P) Compensation: LVN $22-$28/hr | RN $30-$40/hr Major Responsibilities Provide 1:1 skilled nursing care in accordance with physician orders Administer prescribed medications and treatments Monitor patient condition and report changes Collaborate with care team and family Complete accurate clinical documentation electronically Assess home health patients to identify physical, psycho‑social, and environmental needs Implement, develop, and document the plan of care to ensure quality and continuity of care Provide care utilizing infection control measures that protect both staff and patient (OSHA compliant) Comply with all VIVA Pediatrics policies and procedures and promote the agency philosophy Provide effective communication to patient/family, team members, and other health care professionals via clinical notes, case conferences, and evaluations Notify supervising nurse of changes in the client’s condition immediately Provide health education to client and/or caregiver Document interim changes of plan of care in a timely manner Qualifications Active Texas LVN or RN license Current CPR certification Valid Driver’s License At least 1 year of work experience or 6 months pediatric home health as a licensed RN or LVN Benefits Paid Time Off Healthcare Benefits (Medical, Dental, and Vision) In‑house Training Employee Referral Bonus Bonus Incentives Weekly Pay 401(k) Long‑term & Short‑term disability About Us VIVA Pediatrics is a nurse‑owned and operated provider of pediatric home health and medical day care services, committed to delivering high‑quality care to children and their families.
