PRN/Variable opportunity available

Find your life’s work at HopeWest as Social Worker supporting our patients and families in Delta County. This role will work PRN/Variable which offers a unique opportunity for flexible scheduling, having a "mobile office", being out on the road and seeing our patients/families in their personal living environments in this beautiful rural community.

You will be part of an interdisciplinary care team of social workers, chaplains, nurses, physicians, and certified nursing assistants all working together to improve people’s everyday lives when they need help the most. You will be an advocate by providing education and resources to patients and their families.

This role provides social work services, including counseling, group facilitation, and resourcing for assigned patients and families and is responsible for the initial and ongoing assessment of needs, and initial and ongoing development of a Plan of Care in conjunction with the other interdisciplinary team members. The Social Worker plays a key role in adding volunteers to the patient’s care team. Additionally, this role is responsible for the initial assessment of bereavement risk levels of family and friends as well as re-evaluation of levels as needed.

QUALIFICATIONS :

• Master’s Degree from an accredited social work program preferred. Bachelor’s degree from an accredited social work program required and will have additional oversight from an MSW.
• If Master’s prepared, obtains and maintains Colorado licensure as a Licensed Social Worker (LSW), or Clinical Social Worker (CSW) with the State of Colorado Department of Regulatory Agencies (DORA). If not licensed at time of hire, will be required to obtain within 90 days.
• Minimum of one (1) year supervised experience in the health care field or during a supervised internship.
• Valid Colorado driver’s license, adequate auto insurance, and unlimited access to reliable transportation maintained throughout employment.
• Able to work in a variety of environments.

COMPETENCIES & REQUIREMENTS :

• Computer proficiency in Microsoft products (Word, Excel, and Outlook) and EMR.
• Ability to cope with patient/family emotional stress within the framework of the hospice philosophy.
• Participates in the on-call, holiday, weekend, and after-hour’s rotation.
• Ability to function as an effective member of the interdisciplinary team.
• Effective communication, problem-solving customer relation skills to build rapport with facility staff members to enhance quality care to patients.
• Ability to interact appropriately with geriatric populations and clients with cognitive behavior impairment and other various barriers to communication.
• Demonstrates a driven curiosity for knowledge of available community resources for patients/families.
• Current CPR certification is maintained throughout employment.

Pay and Benefits:

• $27.18 - $39.00/Hour, plus any shift differentials that may apply.
• Premium pay for Licensed Clinical Social Worker and/or bilingual fluency
• Mileage reimbursement of 65cents/mile for work related driving

  Across Delta County: HopeWest Social Workers provide care in patients' home settings. When a patient lives in a nursing home or assisted living residence, they partner with professional colleagues in those settings to provide care.
