Clinical Liaison

Job Summary:

As a Clinical Liaison, you will play a critical role in building relationships with healthcare professionals and facilitating the transition of patients from acute care settings to our skilled nursing facility. Responsibilities:

• Develop and maintain relationships with healthcare professionals, including hospital case managers, discharge planners, and physicians to facilitate the transfer of patients to our skilled nursing facility

• Conduct interviews with patients and family members to assess their needs and preferences, and develop individualized care plans to meet those needs

• Collaborate with the interdisciplinary team to ensure seamless transition of patients and effective communication with healthcare providers

• Participate in care conferences and other meetings to ensure that patients' needs are being met and to identify opportunities for improvement

• Provide education and support to patients and families on our services and benefits

• Identify and address any barriers to patient care and work with the team to resolve issues

• Maintain accurate and timely records of patient interactions and communication with healthcare professionals
