MAT Licensed Practical Nurse

Sun River Health is currently seeking a full-time MAT Licensed Practical Nurse to join our team in Coram, Long Island, NY.

Position Summary

The MAT License Practical Nurse is a part of multidisciplinary team within that uses the nursing process to plan, implement, and evaluate patient care and the use of resources. This position will monitor quality of care to ensure that infection control, risk management and all other aspects of services are appropriate. Additionally, this person will serve as patient advocates, community resource referral coordinators, provides crisis management, and understand patient health status. This candidate should understand harm reduction approaches and must demonstrate empathic understanding of medical and psychosocial development needs in addition to having strong crisis management skills.

Job Type: Full-Time

Pay Range: $29.00 - $33.02 per hour

Sign-On Bonus: $2,700 Sign-On Bonus for Eligible External Applicants – Terms and Conditions Apply.

Essential Duties and Responsibilities

• Interviews and maintains patient’s medical record to include: nursing observation; health history to include past and present use of medication; vital signs; vision/hearing tests; medications and treatments given; Reactions to tests i.e. PPD measurements; laboratory test results, utilizes patient identifiers. (PEAs, PhQ, Pain, Smoke Status, etc.) CDSS and ensure that CVRs are completed as needed
• Perform in-house labs, collect specimens for out-side labs perform quality control in accordance with organizational protocols.
• Prepares equipment and assists physician during treatments and examinations of patients.
• Complete and update all assigned tasks & logs (i.e. correctly recording shipments and vaccines received in appropriate areas, signing out meds & treatments in log).
• Initiates follow-up as necessary (as directed by provider) i.e. encounter, labs, close non-billables, etc
• Does health teaching as indicated by physician, diagnosis and/or inquires by patient. Directs patient to Registered Nurse as necessary.
• Will collect data on patients under supervision of the Registered Nurse or provider. May not triage; may not interpret patient clinical data or act independently on such data.
• Maintain MAT roster & MAT case conference spreadsheets.
• Complete Prior Authorizations for specialized medication and services.
• Attend case conferences and document case updates in patient record.
• Collection of urine drug screens.
• Follow up on patient adherence and document accordingly in patient chart.
• Complete phone screenings on potential MAT patients.
• Collaborate with community and referral resources
• Attends ongoing MAT related training
• Participate in quality improvement measures to ensure chart compliance and best practice.

Requirements

• Minimum Education: High School Diploma/GED
• Preferred Education Requirement: Vocational/ Technical School/ Diploma Program - Nursing
• Minimum Work-Related Experience: 1 year
• Preferred Work-Related Experience: 2-4 years
• Driver’s License
• Minimum Licensure:
  • Valid New York State LPN License
  • CPR Certification
  • Infection Control Certification
