MDS Coordinator (RN) - Join Excel Care Newark's Team!

We're seeking a talented MDS Coordinator (RN) to join our team and help us achieve our mission.
As a MDS Coordinator (RN) , you will play a vital role in our care team, coordinating the Minimum Data Set (MDS) process, ensuring accurate and timely data collection, and providing exceptional patient care.
Responsibilities:
Coordinate the MDS process, ensuring accurate and timely data collection
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Monitor patient progress, identify areas for improvement, and make adjustments to care plans as needed
Provide education and support to patients, families, and healthcare team members
Maintain accurate and up-to-date patient records
Ensure compliance with regulatory requirements and company policies

Current RN license in the state of Delaware
Experience as a Registered Nurse
