Nurse Practitioner / Physician Assistant

Position Overview Seeking a team-oriented Nurse Practitioner or Physician Assistant to join a multi-specialty clinic in Austin, TX. The role involves providing primary care services, demonstrating safe clinical decision-making, and collaborating with the healthcare team to deliver exceptional patient care aligned with a transformative payvider model. Responsibilities Interview members, perform physical exams, order tests, prescribe medications, and recommend treatments Manage continuity of care for patients with long-term conditions Make referrals to secondary and tertiary care as needed Perform medical procedures within privileges Conduct health maintenance visits, immunizations, screenings, and health education Document all contacts accurately in electronic medical records Participate in team meetings, clinical committees, and quality initiatives Qualifications & Skills Graduate of an accredited Physician Assistant or Nurse Practitioner program and certified accordingly Current licensure and certification to practice in Texas, with family medicine credentials DEA and CPR certifications, valid Texas Driver's License Comfortable working independently in a clinic setting Proficient in electronic health records, with excellent communication and customer service skills Effective teamwork and strong judgment with confidentiality Preferred Over 2 years' experience in primary care Bilingual in English and Spanish Physical Requirements Ability to work in a moderate-stress environment, handle exposure to fluids and diseases Lift up to 50 pounds, perform patient care activities requiring full body motion #J-18808-Ljbffr Northshore Home Health Care
