Work Environment This position is primarily onsite, enabling you to deliver hands-on, community-based care. You will work in a home care setting, which allows for a personal touch and direct engagement with residents in their familiar surroundings. Key Responsibilities Conduct thorough evaluations of each resident's physical capabilities, identifying specific treatment needs through assessments. Design and implement customized treatment plans aimed at improving strength, mobility, and overall functional independence based on individual evaluations. Administer therapeutic exercises, manual therapy, and advanced modalities like electrical stimulation and ultrasound to address specific physical impairments. Continuously monitor and document patients' progress, adjusting treatment plans in real time to ensure their best possible outcomes. Engage in interdisciplinary collaboration, working with physicians, nurses, and occupational therapists to create a comprehensive care plan for each resident. Educate residents and their families about their conditions and provide guidance on home exercise programs to promote ongoing wellness. Stay informed about state and federal regulations, ensuring that all practices comply with the highest professional standards. Patient Population / Client Focus At Strengthen Physical Therapy, our primary focus is on the geriatric population, assisting residents who might experience age-related physical conditions or chronic illnesses. You will work closely with individuals who require rehabilitation due to surgery, injury, or progressive conditions, utilizing your skills to enhance their physical well-being. This personalized approach ensures that each resident receives attention and care tailored to their unique circumstances, fostering a supportive recovery environment. Qualifications A current license as a Physical Therapist in New York State is mandatory. A Bachelor's Degree in Physical Therapy is required; advanced degrees (Master's or Doctorate) in related fields are encouraged. Strong communication and interpersonal skills for effective patient interactions and team collaboration. Excellent organizational skills to maintain documentation and mirroring treatment plans efficiently. Preferred Experience At least two years of experience as a Physical Therapist, particularly in home care settings or skilled nursing facilities.
