Key Responsibilities

• Provide skilled physical therapy services to home health patients in Medford, OR and surrounding areas
• Conduct patient evaluations, develop individualized treatment plans, and deliver hands-on therapeutic interventions
• Complete OASIS assessments and maintain accurate clinical documentation using EPIC
• Manage a caseload of up to 6 patient visits per day, meeting standard productivity expectations
• Coordinate care with the broader home health team to support patient goals and outcomes
• Travel independently to patient homes within the service area, with an occasional radius of 50+ miles

Required Qualifications

• Minimum 1 year of skilled home health physical therapy experience
• Active or pending Physical Therapist license in the state of Oregon
• Current Basic Cardiac Life Support (BCLS) certification
• Demonstrated experience with OASIS documentation
• COVID-19 primary vaccination series plus one booster required
• Reliable personal transportation required

Preferred Experience / Skills

• Familiarity with EPIC electronic health records system
• Experience managing productivity point expectations in a home health setting
• Comfort navigating a geographically distributed patient caseload
