We are hiring a Speech Language Pathologist (SLP) for a Home Health position providing skilled care to patients in their homes in the Collegeville, PA area. This position is eligible for a $5,000 first year success bonus to reward the accomplishment of key achievements during your first year!

What You’ll Do:

• \n

  Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

\n
• \n

  Perform comprehensive patient evaluations assessing speech, language, cognitive-communication, and swallowing function, developing individualized treatment plans to support safety, independence, and quality of life

\n
• \n

  Educate and empower patients and caregivers on therapeutic exercises

\n
• \n

  Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

\n
• \n

  Work independently, but never alone

\n
• \n

  Travel to patient homes within assigned territory

\n

SLP Qualifications

\n

\n
• \n

  Active Speech Language Pathologist, SLP license in the state you work

\n
• \n

  One year of experience as a Speech Language Pathologist and the clinical expertise to deliver quality patient care

\n
• \n

  Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

\n

\n

\n
