Responsibilities:

• Assist physical therapists in the development and implementation of individualized treatment plans for our residents

• Conduct physical therapy treatments, such as exercise programs, modalities, and other therapeutic interventions

• Communicate effectively with patients, families, and other healthcare team members to ensure seamless care coordination

• Maintain accurate and detailed records of patient treatments and progress
