Requirements Locums | Medford, OR | Invasive Cardiology (General) ________________________________________________________________ SCHEDULE • Coverage Type: Inpatient/Outpatient • Schedule Type: Hospital onsite, Clinic and Call • Call Type: Weeknight Beeper and 24 Hour Weekend Beeper Call • Dates of Need: we need 1-2 weeks of coverage (with coverage of 1-2 weekends). Dates can be flexible but will need to work with the clinic to ensure they fit for all. • Shift Times: 8am-5pm onsite (will be on call which is included in hourly time). 5pm-8am will be on beeper call. This will be billed at the regular hourly rate and the night call rate on the weekdays. On weekends provider will be on beeper call (billed at the 24hr call rate). • Lunch N/A • Provider Min # Shifts Required: 7 -14 shifts a month • Guaranteed Hours per Day: 8 • Hours Worked for OT to apply: 9 hours per day • Rates to be Filled Out on Bid: Clinic and Call: Regular, Orientation, Overtime, Holiday, Night Call, 24 Hour Call, Call-Back. ________________________________________________________________ CREDENTIALING • Current CV (must be uploaded to bid) • License: Active • DEA Required: Yes • Boards: BC only • Skills/Certs: BLS, ACLS • Other: ________________________________________________________________ ADDITIONAL DETAILS Providers will be contacted by the lead physician by phone for a screening, they will then be scheduled for a teams interview with the other team members for review. Please provide their availability in the next two weeks to go through this interview process. Looking for providers with experience with the following: - Cardioversion - TEE's -Stress Echo's - Reading echoes, EKG's and stress echo's Nuclear reads are preferred but not required they do have someone onsite that can do it.
