Hourly Pay Range $57.79 - $79.76 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. Position Highlights Position: Advanced Practice Provider (APN/PA) – Center for Cardiac Health - Heart Failure. Location: Edward Hospital, Naperville, IL. Full Time/Part Time: Full-Time (average of 40 hours/week) Hours: Monday – Friday between 7:00 AM – 5:30 PM. Call in the future may be required. Required Travel: Possible travel to Elmhurst Specialty Clinic may be required. Outpatient role. Job Description Exciting opportunity to Join Endeavor Health’s Center for Cardiac Health–Heart Failure team in an autonomous role delivering patient-centered care to high‑acuity patients under the direction of a collaborating physician. Key Responsibilities Obtaining comprehensive patient histories and performing physical examinations Ordering and interpreting diagnostic tests Developing differential diagnoses and prescribing evidence‑based pharmacological and nonpharmacological treatments Managing acute and chronic conditions Providing ongoing education to patients and their families Coordinating care with physicians and interdisciplinary teams to optimize outcomes and support post‑discharge follow‑up and transitional care Supporting quality‑improvement and patient‑safety initiatives Contributing to the development and implementation of organizational and service‑line goals Adhering to all Endeavor Health policies, procedures, and standards.
