Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive physical therapy assessments, including OASIS evaluations, in patient homes.
• Develop and execute individualized care plans focused on mobility, strength, and safety.
• Deliver interventions such as therapeutic exercise, gait and transfer training, and neuromuscular re-education, with a caseload of approximately 2532 points per week (about 57 visits per day).
• Monitor patient progress, adapt therapies as needed, and communicate status changes to physicians and interdisciplinary team members.
• Provide patient and caregiver education on exercises, safety, home adaptations, and assistive device usage.
• Use Home Care Home Base (HCHB) for accurate, timely documentation.
• Participate in case conferences, team meetings, and continuing education as required.

Qualifications:

• Active Physical Therapist (PT) license, in good standing.
• At least one year of recent field-based home health physical therapy experience.
• Proficiency in OASIS assessments (required).
• Familiarity with Home Care Home Base (preferred).
• Exceptional clinical, organizational, and communication skills.
• Reliable transportation, valid drivers license, and current auto insurance.

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
