Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM

Qualifications:

• Active, unrestricted Physical Therapist (PT) license in Indiana
• Minimum one year of recent home health physical therapy experience required
• Demonstrated proficiency with OASIS assessments
• Experience with Home Care Home Base (HCHB) electronic documentation preferred
• Excellent functional assessment and clinical decision-making skills
• Ability to work independently while effectively managing a dynamic caseload
• Reliable transportation, valid drivers license, and current auto insurance
• Strong organizational, communication, and time management abilities

Key Responsibilities:

• Conduct comprehensive physical therapy evaluations, including OASIS as needed for start of care, recertification, and discharge
• Develop and implement individualized treatment plans focused on restoring mobility, improving strength and balance, and enhancing safety
• Deliver skilled therapeutic interventions such as gait and transfer training, balance activities, pain management strategies, and fall prevention
• Provide ongoing education to patients and caregivers on exercises, safety, and use of assistive devices
• Accurately document care provided using Home Care Home Base
• Collaborate closely with nurses, occupational therapists, speech therapists, social workers, and home health aides
• Attend interdisciplinary meetings and participate in ongoing professional education

$2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
