KAW NATION POSITION DESCRIPTION

Title: BEHAVIORAL HEALTH CARE NAVIGATOR Immediate Supervisor: HEALTH DIRECTOR Controlling Supervisor: Chair Status: Regular, full-time Location: Newkirk, OK

QUALIFICATIONS

Requires a High School Diploma or GED with a minimum of 2 years’ experience in a clinical/physician office setting. Possess interpersonal skills in reading, writing, and speaking which would include good communication skills. Must be able to multi task and work with minimal supervision. Must have proficient computer skills. Knowledge of comprehensive health programs, standards, practices, and procedures.

RESPONSIBILITIES AND DUTIES

Answer phones, check patients in and out, and schedule appointments. Review patient registration data to update and correct patient information ensuring all information is correct. Schedule, coordinate, and facilitatereferrals to and from facility. Ensure Clinic and outside facilities receive test results, reports, notes, and documentation of care. Monitor and evaluateservices, regularly contacting patients to track progress and identify new needs. Secure pre-authorizations of outside care, as required by 3rd party insurers. Act as a liaison between patients, their families, providers, and other staff for continuity of care. Coordinate servicesacross multiple agencies, building strong working relationships. Establish and maintaintherapeutic relationshipswith patients and their families. Excellentinterpersonal and communication skillsfor in-person and telephone interactions. Ability towork efficiently under pressureand meet deadlines consistently. Observe strict confidentiality in all matters, responsibilities and duties performed under this position and adhere to the HIPAA (Privacy Act) in regards to patients’ medical information.
