Position Overview 

AMS staffs a wide range of home health and community care roles, including but not limited to: 

• Nursing & clinical care 

• Home Health RNs, LPNs, and LVNs 

• Therapy & rehabilitative services (PT, OT, SLP) 

• Behavioral health & counseling 

• Home Health Aides and CNA support 

• Hospice care 

Assignments are available in patient homes and community-based settings, depending on care needs and candidate qualifications. 

Assignment Details 

• Locations: Nationwide 

• Setting: Home health, hospice and community-based care 

• Pay Range: Weekly pay based on role, experience, and location 

• Schedule: Full-time and part-time opportunities available; schedules vary by assignment 

• Contract Length: Short-term and long-term assignments available 

• Candidate Types: Local and travel candidates considered 

Requirements 

• Active state license, certification, or credential, as required for your discipline 

• Ability to meet all agency, state, and federal compliance requirements 

• Prior home health experience preferred, but not required for all roles 

Responsibilities (Role-Dependent) 

• Provide care to patients in home and community settings in accordance with care plans and scope of practice 

• Assess, monitor, and support patient needs in a home-based environment 

• Maintain accurate documentation in compliance with regulatory and agency standards 

• Collaborate with interdisciplinary care teams to support patient outcomes 

• Educate patients and caregivers on treatment plans and ongoing care
