Licensed Master Social Worker - Equiliem

Job Description:
Conduct psychosocial assessments and supportive therapy as well as discharge planning that includes making referrals.

Education Requirement:

• Required Education: Master's Degree in Social Work.
• Required Certifications & Licensure: Valid Licensed Master of Social Work (LMSW) license issued by NYSED, Primary Source Verification.

Knowledge, Skills, Abilities:

• Required Skills & Experience: One (1) year of LMSW experience.
• Schedule Notes:
  • Inpatient medicine, surgery, (med/surg), rehab experience
  • Manager would like someone with Hospital experience
  • At least 1 year of experience
  • Required Education: Master's Degree in Social Work
  • Valid Licensed Master of Social Work (LMSW) license issued by NYSED required
  • 9:00 AM-5:00 PM
