SCOPE OF POSITION: The Pathway of Hope (POH) is a Salvation Army initiative offering strengths-based case management to families seeking to break the cycle of poverty and achieve long-term stability. The Social Services Coordinator plays a key role in leading social service and POH efforts at the Tri-Cities Corps by assessing client needs, developing individualized action plans, and connecting families to housing, health, financial, and emotional resources. Using motivational interviewing and trauma-informed care, the Social Services Coordinator works closely with corps staff and community partners to build strong support networks and promote self-sufficiency. This role includes maintaining accurate records through WellSky Community Services, ensuring culturally competent and ethical care, and fostering collaboration with other service agencies in the Tri-Cities area. EDUCATION AND WORK EXPERIENCE: High school degree or GED required Associate's degree (AA) in Social Work, Social Services, or a related field preferred - or a strong desire to learn and grow in the role. Minimum 1 year of experience working with low-income or homeless families, Case Management experience preferred. LICENSING & BACKGROUND Must possess a valid Washington State driver's license and be 21 years of age or older. The Salvation Army Protecting the Mission (PTM) training and certification is required within 90 days of hire. KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: Detail oriented General understanding of homelessness, trauma, and mental health challenges. Familiarity with Strengths-Based Perspective, Stages of Change Model, and Motivational Interviewing is a plus. Ability to work compassionately with diverse populations, especially those impacted by poverty and crisis. Ability to work independently and as part of a team. Ability to read, write, speak, and understand English; bilingual English/Spanish strongly preferred. Excellent telephone skills Ability to work in a fast-paced environment and maintain poise under pressure Customer service mindset Strong interpersonal communication, organization, and problem-solving skills. Strong analytical and research skills Proficient typing skills (40 words per minute) General knowledge of business protocols Ability and willingness to keep information confidential SOFTWARE-RELATED SKILLS: Microsoft Office Suite Working knowledge of integrated database applications and ability to use new software programs with basic training. ESSENTIAL DUTIES AND RESPONSIBILITIES: Community Program Coordination Coordinate logistics, preparation, and distribution for Angel Tree, Back to School, and other seasonal community programs. Drive a TSA vehicle to transport supplies to and from seasonal community programs. Plan and implement appropriate levels of social service support throughout the Tri-Cities area. Client Services & Case Management Screen clients and complete applications for Salvation Army programs, including Pathway of Hope (POH). Determine eligibility and conduct intake assessments (e.g., URICA, Client Sufficiency Matrix, Herth Hope Index). Develop and monitor individualized action plans and goals. Provide ongoing case management at 30, 60, and 90 days and 12 months, with one year of follow-up after POH completion. Offer referrals to community resources (housing, health, benefits, financial support, etc.). Respect client confidentiality and adhere to HIPAA, 42 CFR, and POH guidelines. Data & Reporting Accurately record and maintain client data using WellSky Community Services. Compile and submit required monthly statistical and service reports. Track and report all services provided. Teamwork & Training Participate in all required POH and WellSky trainings, including Strengths-Based Perspective, Motivational Interviewing, Trauma-Informed Care, and De-escalation. Attend internal case management meetings and coordinate with the Divisional POH Coordinator. Maintain strong relationships with internal teams and community partners. Keep updated directories of available community resources. Additional Duties Support clients' spiritual growth by connecting them with pastoral care staff. Drive Salvation Army vehicles to attend meetings or conduct outreach, as needed. Prepare agendas, record and distribute minutes Maintain supply inventory and order replacement supplies as necessary Prepare general correspondence as requested, including form letters Provide administrative support to Business Coordinator Perform other administrative functions as requested PHYSICAL REQUIREMENTS: Ability to sit, walk, stand, bend, squat, climb, kneel, and twist on an intermittent or continuous basis Ability to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead Ability to operate telephone Ability to operate a desktop or laptop computer Ability to lift up to 25 lbs. (usually file boxes) Ability to access and produce information from a computer Ability to understand written information Qualified individuals must be able to perform the essential duties of the position with or without accommodation.
