Conduct thorough evaluations of patients in inpatient or outpatient settings and create tailored treatment plans
Implement physical therapy interventions with a focus on enhancing mobility, strength, balance, and overall functionality
Engage in collaborative efforts with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
Educate patients and their families on therapeutic objectives, discharge plans, and home exercise programs
Accurately document evaluations, treatments, and patient progress in alignment with facility protocols and regulatory standards
Participate in care team discussions and interdisciplinary meetings to ensure cohesive patient care
Doctorate in Physical Therapy (DPT) or appropriate equivalent from an accredited institution
Active state licensure as a Physical Therapist
CPR certification is a preferred qualification
Any specialized certifications (like neurological or geriatric) are advantageous but not mandatory
Prior experience is highly preferred
Capability to manage patients facing complex medical and mobility challenges
Strong teamwork and communication skills are essential
Proficiency with digital charting and clinical documentation systems
