Schedule:

• 13-week contract with full-time hours
• Monday through Friday, with probable on-call requirements
• Structured daily patient visit expectations

Key Responsibilities:

• Deliver in-home nursing care and skilled interventions to patients
• Complete thorough OASIS assessments and timely, accurate documentation
• Work collaboratively with physicians and interdisciplinary care teams for optimal patient outcomes
• Perform wound care, medication management, and chronic disease education
• Provide patient updates and condition changes to the clinical team promptly

Qualifications:

• Active, unrestricted RN license
• Minimum of one year of recent home health nursing experience
• Expertise in OASIS documentation is mandatory
• Experience with HomeCare HomeBase (strongly preferred)
• Proven ability to manage a structured caseload and full-time schedule
• Reliable transportation and valid drivers license
