Field Nurse (LPN): In-Home Health & Wound Care
Wellbound Home Care
Wellbound Home Care is seeking a Field Nurse to provide compassionate, proactive nursing care in assisted living facilities. You will assess patients, develop individualized care plans, and administer medications as prescribed. You will educate patients and families on health management, coordinate with interdisciplinary teams, monitor progress, and adjust care plans as conditions evolve. Travel between patient homes is required, including insulin injections and wound care. #J-18808-Ljbffr Wellbound Home Care
