Engaging patients and providers through a data-driven approach, the full-time Hybrid Population Health Specialist will implement and optimize value-based care workflows while supporting population health management initiatives remotely in Bedford, NH. Key responsibilities Support the Population Health Manager in implementing provider engagement initiatives at practice or clinic levels Contribute to improving provider experiences with value-based care through practice-based support and coaching Coordinate patient health promotion efforts and manage care gaps to enhance population health outcomes Required qualifications Bachelor's degree in Healthcare Administration, Business Administration, or a related field 1+ years of healthcare experience in roles such as health coach, medical assistant, or patient navigation Ability to thrive in a fast-paced and dynamic environment Effective communication skills for interaction with multi-disciplinary teams and patients Strong organizational skills with a focus on meeting timelines and goals
