Job Summary:

Provides comprehensive physical therapy services to patients, focusing on restoring and improving physical functions through individualized treatment plans. Collaborates with interdisciplinary teams to enhance patient care and outcomes.

Responsibilities:

• 
  Conducts detailed evaluations to assess patient needs and establish customized treatment goals.
  
  
• 
  Implements evidence-based therapy interventions using appropriate clinical techniques and protocols.
  
  
• 
  Develops and communicates individualized care and discharge plans to patients and team members.
  
  
• 
  Supervises assistant staff and ensures clinical documentation is accurate and completed within deadlines.
  
  
• 
  Performs ongoing assessments to monitor progress and adjust treatment strategies accordingly.
  
  
• 
  Ensures compliance with all licensure, certification, and organizational policies.
  
  

Qualifications:

• 
  Holds a Doctor of Physical Therapy (DPT) degree, obtained or licensed after January 2016, or equivalent foreign credential.
  
  
• 
  Maintains current physical therapy licensure in all applicable states of practice.
  
  
• 
  Possesses valid CPR/BLS certification from an approved provider.
  
  
• 
  Demonstrates at least two years of recent experience in the field, including home health, with proficiency in HCHB, OASIS, and EPIC.
  
  
• 
  Is at least 21 years old with a valid drivers license and clean driving record.
  
  
• 
  Complies with vaccination requirements and participates in E-Verify.
